While dermatology and skin are the focus of Dr. Brooks' practice, it is the person inside the skin that matters most. No matter if Dr. Brooks has seen your condition on 1,000 other patients he understands that your issue is unique to you. He'll take the time to discuss your concerns, examine the condition, make an expert diagnosis based on nearly his vast dermatological experience and suggest a course of treatment. He'll explain what he's doing and why so that you can always be comfortable and confident that you're getting the best possible care from Dr. Brooks.

Dr. Ralph A. Brooks strives to maintain the highest level of patient care possible. His goal is to offer the most innovative and least invasive treatment options available while making our patients feel at home and as comfortable as possible. We value the relationships we've formed with our existing patients over the years and we look forward to serving you as well.

New Patients

If you are new patient of Dr. Brooks' or are considering becoming a patient, we want to make the process as easy as possible for you. If you have not arranged for your initial appointment, you can do so either by completing the Appointment Request form on this page or by calling us during normal business hours at 817-870-9718.

Prior to your first appointment, please download and complete the Patient Packet and bring it with you. If you are unable to print this document, please plan on arriving at least 15 minutes prior to your appointment to complete it at our office. You'll need Adobe Reader to view this document.

In addition, please bring the following:

• Your insurance card and photo identification card
• Copayment / payment for your visit. Your insurance may require us to collect a copay at the time of your visit.
• Physician referral forms if required by insurance.
• List of current prescription and any over-the-counter medications you are taking including how much (dose), and how often (frequency) you take them.

If you do not see your insurance company or plan listed here, please contact your insurance company to obtain a "preferred provider" list to make sure Ralph Brooks, M.D. is a "participating provider" on your plan prior to scheduling an appointment. Some plans may require you to obtain a referral from your primary care provider prior to seeing a specialist.

Existing Patients

If you have a question or concern, please call us during normal business hours at 817-870-9718. After hours, please leave you name, a contact number and a brief message on our answering system and your call will be returned the next business day. If it's an emergency call 911!
